Request Diploma

Graduates can request additional diplomas by completing the Supplemental Graduation Items Request form and uploading it here. Alumni students will be required to pay $5.00 and can pay over the phone by calling (602) 286–8277 or in person.

Note: If you do not know your MEID, please click here for assistance in accessing your account and resetting your password (if necessary).

Follow these steps:

1. Complete the Supplemental Graduation Items Request Form
2. Upload the Supplemental Graduation Items Request Form
3. Once approved by the Admissions and Records Department, Alumni will be contacted via email and/or phone to pay and arrange delivery or pick up options
